Small intestinal transplantation for irreversible intestinal failure in children.

چکیده
Permanent intestinal failure, whether secondary to severe gastrointestinal tract dysfunction or short bowel syndrome, remains a challenging pediatric health problem that requires innovative therapeutic modalities. Steatorrhea and intractable diarrhea result in failure to thrive, hypovitaminosis, hypoproteinemia, electrolyte disturbance, and metabolic acidosis if parenteral alimentation (TPN) is begun too late. A variety of medical and surgical treatments have been devised to address each of the underlying causes of childhood intestinal failure. Nonetheless, a significant percentage of children with intestinal failure remain permanently bound to parenteral nutrition. While some of these children suffer minimal morbidity, others succumb to the life-threatening complications of TPN. Until recently they had no hope for survival, but within the past decade small intestinal transplantation has offered them an opportunity for prolonged life emancipated from the shackles of TPN. In this review, we shall discuss in detail microvillus inclusion disease. intestinal pseudoobstruction. and short bowel syndrome. which are the most common causes of pediatric intestinal failure coming to intestinal transplantation. The factors facilitating intestinal adaptation or predisposing to irreversible liver disease in children with short bowel syndrome will be described. We shall also enumerate the indications

Isolated liver transplantation for treatment of liver failure secondary to intestinal failure
Intestinal Failure is a permanent loss of digestive and absorptive functions as a consequence of short bowel syndrome and/or other primary intestinal conditions. Patients with intestinal failure (IF) require long term parenteral nutrition to survive. The only alternative to parenteral nutrition is intestinal transplantation which still entails high mortality. Children with intestinal failure on...
متن کاملRecent progress in intestinal transplantation.
Transplantation of the kidney, liver, or heart is now a well established and successful treatment for irreversible organ failure. It has been much more difficult to achieve success in intestinal transplantation, although experimental and clinical work has been carried out in this field since the late 1950s. However, in the last three years there have been several successful human small bowel tr...

Intestinal transplantation has emerged in the last decade as a lifesaving procedure for patients with intestinal failure who are suffering from complications arising from the administration of total parenteral nutrition. Indications for transplantation include irreversible liver injury and loss of vascular access. At least 50 children in the United States may benefit from intestinal transplanta...
متن کاملIs intestinal transplantation now an alternative to home parenteral nutrition?
Patients with irreversible intestinal failure and complications of parenteral nutrition should now be routinely considered for small intestine transplantation. Despite attempts for >40 years immunological graft intolerance presented an impenetrable barrier to successful engraftment until the development in the late 1970s of the powerful calcineurin-inhibitor immunosuppressive agents. Their use ...

The advent of tacrolimus allowed clinical intestinal transplantation to become a feasible procedure for patients with irreversible intestinal failure. Over last 5 years, 71 patients underwent intestinal transplantation. Forty-one recipients were children, and 30 recipients were adults. Twenty-five patients received an isolated intestinal graft, 34 patients received a combined liver-intestinal g...
